Sculptra
Sculptra (poly-L-lactic acid) is an injectable biostimulator that gradually rebuilds collagen over time, restoring facial volume and improving skin quality through your body's own regenerative response. It is not a traditional filler — results develop slowly and require a multi-session approach.
Off-label use notice: Some uses of injectable biostimulators, including poly-L-lactic acid (Sculptra), involve off-label applications. Off-label use means the product is being used for a purpose not specifically listed in the FDA-approved labeling, which is common in clinical practice. All treatment recommendations are based on your anatomy, medical history, and clinical judgment.
What Sculptra is
Sculptra (poly-L-lactic acid, PLLA) is an FDA-approved injectable biostimulator. Unlike hyaluronic acid fillers that add immediate volume, Sculptra works by stimulating your body to produce new collagen over time. The product itself is gradually absorbed; it is the collagen your body produces in response that creates the lasting volume and texture improvement. Sculptra is FDA-approved for the correction of facial fat loss (lipoatrophy) in people with HIV and for the correction of shallow to deep nasolabial fold contour deficiencies and other facial wrinkles in immunocompetent patients.
Off-label use
While Sculptra has FDA-approved indications for facial treatment, use in other body areas (buttocks, knees, chest, hands) is considered off-label. Off-label use means the product is being used for a purpose not specifically listed in the FDA-approved labeling, which is common in clinical practice and does not indicate unsafe use. All treatment recommendations are based on your anatomy, medical history, and clinical judgment.
What Sculptra can address
Sculptra is used to address volume loss in the cheeks and temples, deep facial lines and folds, generalized loss of skin thickness and quality, and facial hollowing associated with aging. It is well-suited to patients who want gradual, natural-looking improvement over time rather than immediate visible change.
Who may be a candidate
Adults with mild to significant facial volume loss who want a gradual, longer-lasting collagen-rebuilding approach. Patients should understand that results develop over several months and that multiple sessions are typically required. Sculptra is not ideal for patients seeking immediate visible results or for highly specific structural corrections where a traditional filler would be more precise.
Who should avoid or delay treatment
Sculptra is not appropriate for patients who are pregnant or breastfeeding, have active skin infection or inflammation in the planned treatment area, have a history of keloid scarring or hypertrophic scarring, are allergic to any component of the product, or have certain autoimmune conditions. Patients with a history of granulomatous conditions should discuss this with us before treatment.
What to expect during a session
After consultation and consent, the skin is cleansed and topical or injectable local anesthetic may be used. Sculptra is injected using a fine needle or cannula in a diluted form. Multiple injection points are used across the treatment area. Sessions typically take 30 to 45 minutes. Most patients require two to four sessions spaced four to six weeks apart, depending on the degree of volume loss and desired outcome.
Recovery and downtime
Swelling, bruising, and redness at injection sites are common and usually resolve within a few days. Massage of the treated area is a critical aftercare step — patients are typically instructed to massage the area several times daily for five days after each session to distribute the product evenly and reduce nodule risk. Avoid vigorous exercise and sun exposure for 24 to 48 hours after treatment.
Risks and side effects
Common: temporary swelling, bruising, redness, and tenderness at injection sites. Less common: subcutaneous nodules or papules — small lumps under the skin that can develop if the product is not distributed evenly or is placed too superficially. Most nodules are asymptomatic and resolve over time; some may require treatment. Rare: visible nodules or granulomatous reaction. The massage protocol after each session is important for minimizing nodule risk.
Alternatives
Alternatives for facial volume loss include hyaluronic acid fillers (immediate effect, reversible), calcium hydroxylapatite biostimulators, PRF/PRP protocols, fat transfer, and surgical approaches for significant laxity. Each approach has different onset, duration, and risk profiles. We discuss the most appropriate option at consultation.
How long results last
Results with Sculptra develop gradually over three to six months as collagen builds. Studies suggest results may last two or more years in some patients. Results vary based on age, skin quality, the degree of volume loss being addressed, and the number of sessions completed. Maintenance treatments may extend results over time.
When to contact us
Contact us if you develop a new lump or nodule in a treated area weeks or months after treatment, if a nodule becomes tender or inflamed, or if you are uncertain whether your recovery is progressing normally. Seek emergency care immediately if you develop any signs of vascular compromise after treatment — including skin blanching, significant pain, or visual changes.
Clinical notice
Information on this page is educational and does not replace an in-person consultation. Treatments are recommended only after reviewing medical history, anatomy, goals, and clinical suitability. Results vary. Appropriate candidates are determined at the time of consultation.
Some treatments described on this page include off-label uses. Candidacy requires an in-person exam and medical history review by a licensed provider.
If you experience severe swelling, vision changes, difficulty breathing, signs of vascular compromise, or another urgent symptom after treatment, seek emergency care immediately.
