What this treatment is
Neuromodulator injections into the masseter muscle temporarily reduce muscle bulk and activity. This is an off-label application used for two purposes: aesthetic (reducing the width of the lower face for patients with a prominent masseter) and functional (relieving jaw tension, clenching, and bruxism-related discomfort). These goals often overlap.

Masseter neuromodulator treatment is an off-label clinical use. The product is administered by a licensed clinician based on clinical evidence and judgment. Off-label does not mean unapproved or unsafe; it means the specific indication is not listed in the FDA-approved labeling for the product.
Who may be a candidate
Patients with hypertrophic (enlarged) masseter muscles who are interested in reducing jaw width, patients with clenching or bruxism who experience jaw discomfort, and patients in whom a wider lower jaw is anatomically disproportionate. Masseter size and anatomy are assessed at consultation by palpation. Not every patient with a wide jaw has a prominent masseter — bone structure, fat, and other tissues may be contributing factors that will not respond to this treatment.
Who should avoid or delay treatment
Patients who are pregnant or breastfeeding, have neuromuscular conditions, are allergic to botulinum toxin, have active infection at the injection site, or have not had a dental examination to rule out other causes of jaw discomfort. Patients who rely on significant jaw strength for professional activities (e.g., wind instrument musicians) should discuss impact carefully before treatment.
What to expect during a session
The masseter is palpated to confirm size and location. Injections are placed into the muscle belly, typically in 3 to 5 sites per side. Sessions take 15 to 30 minutes. Mild firmness and soreness at injection sites are common for 24 to 48 hours.
Recovery and downtime
Most patients resume normal activity immediately. Avoid vigorous chewing of very hard foods for 24 to 48 hours. Bruising is uncommon but possible. The aesthetic effect (reduction in jaw width) becomes visible gradually over 4 to 6 weeks as the muscle reduces in bulk.
Risks and side effects
Common: temporary soreness, bruising, or firmness. Less common: asymmetric results, temporary reduction in bite force or chewing efficiency, or an uneven smile if product affects adjacent muscles. With repeated treatment over time, some degree of muscle atrophy may persist longer. We discuss the full risk profile at consultation.
How long results last
Functional effects typically last 4 to 6 months. Aesthetic volume reduction in the masseter may persist somewhat longer than cosmetic facial applications due to the muscle size and dose used. Results vary. Repeat treatment is required to maintain effect. After multiple treatments, some patients notice longer intervals before full muscle activity returns.
Alternatives
For jaw width concerns, alternatives include surgical jaw reduction (osteotomy) for structural bone width or masseterectomy, which are invasive procedures not offered at this practice. For bruxism, alternatives include occlusal splints and behavioral therapy. Neuromodulator treatment is often used alongside, not instead of, other approaches.
